To see if you live in our patient catchment area, please enter your postcode in the box below (under the map) or download map (pdf).
New Patients - If you do live in our catchment area, you can normally register with Wye Surgery (although, very rarely we may refuse a registration request).
Existing Patients - If you move out of our catchment area, you must register with a different practice in your new location (unfortunately, you won’t be able to remain a patient with us).
Each new patient will need to complete 1 or 2 forms depending on age.

Parents, guardians or carers may complete these forms on behalf of a child or incapacitated adult.
If you cannot speak or understand English you can bring a friend or relative into your appointment, to interpret for you. Or if you do not know anyone who can do this, we can arrange for an interpreter to be present.
Please contact us well in advance about this, as we will normally need around 10-14 days notice.

About ‘The UK’s National Health Service (NHS)’: Below are links to information written for newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.
If you are ill while away from home (and are outside the catchment area for your usual GP) or, if you are not registered with a doctor but need to see one, you can receive treatment from us for up to 14 days.
After 14 days you will need to register as either a temporary or permanent patient with us. (If you register as a temporary patient you will also remain a patient of your permanent GP for 3 months. After 3 months you must permanently register with us or go back to the care of your permanent GP.)
To register as a temporary patient simply contact us. We do not have to accept you as a temporary patient although we do have an obligation to offer emergency treatment. You cannot register as a temporary patient if you are already registered with another doctor's practice in our area.

For appointments, patients can see any available doctor of their choice. However, the General Practice contract made by NHS England requires GP practices to nominate a responsible named GP for every patient. We do this based on patient surname, as follows:
Moving? - Please note if you move out of our catchment area, you must register with a practice in your new location (you won’t be able to continue seeing GPs at Wye Surgery).
